I have read, understood and agree to the following information:
PERFORMANCE AND OTHER SHOW DETAILS:
-We at JA Vocal Music work to provide a non-competitive, nurturing and fun environment for our students.
-These workshops are offered to students ages 6 through 12.
-All students will perform 1 solo and 1 group song.
-Any special clothing and/or
costumes are to be provided by the student.
-JA Vocal Music will provide all other materials, such as, rehearsal CDs, sheet music, props, etc. All of these materials must be returned in their original condition at the end of the last ay of camp.
TUITION:
-$390 for the 2-week Summer Sessions (Monday, Wednesday and Friday).
-Enrolled JA Vocal Music
students will receive a discount of 15% off the entire tuition cost.
-Families that have more than 1 student participating in the program will receive a discount of $80 off for each additional student for the Summer Sessions. This applies to immediate family only.
-Those who refer students to the program will receive a $40 credit for the Summer Sessions toward their tuition for each paid referral.
-A $195 deposit for the Summer Sessions is required when turning in the participation form.
-The remaining balance due is to be paid before the first day of the workshop begins.
-There are no refunds for the deposit or tuition payments. There are no refunds if the student is absent due to illness, withdraws early or is removed from camp due to behavior or discipline problems.
-All payments must be cash or check. Checks should be made payable to: JA Vocal Music. If a check payment is returned for non-sufficient funds, there will be a charge of $25. Credit Card payments will not be accepted for our camp programs.
WORKSHOP HOURS AND ATTENDANCE:
-The 2-week Summer workshops will be held from July 1-12, July 15-26 and July 29-August 9, Mondays, Wednesdays and Fridays.
-Regular rehearsal times will be from 9:00am to 3:30pm. All students will be expected to attend all rehearsals.
-Any conflicts with the rehearsal and performance schedule must be submitted in writing by the beginning of the first rehearsal. There will be no refunds for missed rehearsals.
-There will be 1 performance, which will be held outside of these rehearsal times. The performances will be held on the Saturday after the last day of the session (time and location to be announced). All Students must be at the performance. They will be expected to arrive 1 hour prior to the performance. A complete schedule will be handed out on the first day of the workshop.
-Tickets for the performances
will be $5 each. Tickets will not be sold at the door.
-The student will not be allowed to leave the workshop without a parent, legal guardian or an adult that has been registered with JA Vocal Music as an authorized caretaker.
SNACKS AND LUNCH:
-Students are responsible for bringing their own snacks, lunches and drinks.
-There will be a 30-minute lunch break during each camp day. Students should bring their own sack lunch.
EMERGENCY INFORMATION:
-In the case of an emergency, the emergency contact listed on this form will be contacted and, if necessary, 911 will be called.
-By signing this document, you give permission for the emergency technicians and/or doctors to treat the child listed on this form.
-Please provide JA Vocal Music with any special needs or allergies that the student may have.
PROMOTIONAL INFORMATION:
-By signing this document, you give permission for the child listed on this form to appear in any promotional pictures, audio recordings and/or videos for JA Vocal Music.
